The Basics of Root Cause Analysis
Root Cause Analysis (RCA) is a fundamental process within the realm of problem solving. In this presentation, multiple facets of RCAs will be discussed, with a focus on the “5 whys” technique. Other methods will be discussed briefly.

The purpose of the presentation is to provide the audience with insights on the powerful, positive attributes of structured problem solving using the 5 whys technique. Another important purpose is to provide the audience members with techniques and resources to enhance their skill set.
About Alan
Alan Piciacchio joined IBM in 1991 after receiving BS and MS degrees in Materials Science and Engineering from Penn State. During the first part of his career, he was a Manufacturing Engineer in Microelectronics Division (MD), focused on component quality, yield optimization, problem solving, and industrial engineering projects. During the second part of his career he was a manager in MD, in IBM’s multi-layer ceramic business line then in the 300MM semiconductor fab. In 2008, he went back to his engineering roots by transferring into IBM’s Global Technology Services (GTS) division, in various roles as a Quality Engineer and an Industrial Engineer. He is currently pursuing an appointment of Distinguished Engineer. When not hammering away at RCAs, Alan can be found spending time with his family. He also regularly runs and hikes and occasionally hacks his way through coaching and playing soccer.
